Welcome to Pearlman Aesthetic Surgery for eyelid surgery in NYC on Park Avenue.

Also known as blepharoplasty, eyelid surgery can correct a multitude of concerns around the eye area. Removing excess skin, fat, and muscle from the upper or lower eyelids results in a younger looking, refreshed appearance as well as improved vision for some patients whose eyelids drooped far enough to impair it.

If you feel bothered by the way your eyes are aging, eyelid surgery in NYC can help you restore your youthful appearance.

Creating a Youthful, Wakeful Look

The eyes are often the first feature noticed on our face—and they are also one of the first areas that truly reflect our age. As we get older, the skin and muscles surrounding our eyelids begin to loosen and sag.

Eyelid surgery, also known as blepharoplasty, can effectively restore a youthful look to your eyes and improve the overall attractiveness of your face. If you feel bothered by eyes that appear to be aging, it’s time to explore the options of blepharoplasty with Dr. Steven Pearlman, a dual board-certified facial plastic surgeon in NYC.

Anyone who is bothered by sagging eyelids, puffy eyes, discoloration or undereye bags may be an excellent candidate for a blepharoplasty. Dr. Pearlman suggests that patients be in good mental and physical health, with no major medical issues that might interfere with surgery or its recovery. Patients should be non-smokers, and have realistic expectations for improvement based on their confidential consultation at our Manhattan office.

Recovery

To prepare for your blepharoplasty in NYC, you will first meet with Dr. Pearlman for a consultation at our office on Park Avenue. Dr. Pearlman will listen to your concerns and desired results from this procedure .

He will then examine the treatment area, explain potential options for the results that will best suit you, and answer any questions you may have.

Whether you’re treating tired eyes, drooping eyelids, or another concern, your blepharoplasty / eyelid surgery in NYC with Dr. Pearlman will have natural, rejuvenated results.

Blepharoplasty is an outpatient procedure. An upper blepharoplasty is often performed using only local anesthetic. For a lower blepharoplasty, or if we’re combining with another procedure like a brow lift or facelift, you’ll be given general anesthesia.

To make blepharoplasty scars inconspicuous, Dr. Pearlman makes incisions in the natural creases of the eyelids and sometimes inside the lower eyelid to hide them.

The surgery will take 1 to 2 hours depending on your specific eyelid surgery. Treating 1 eyelid takes only about 1 hour, if you’re treating both your procedure will take more time.

Be sure to have someone available to drive you to and from your surgery.

As with all of our surgical treatments, our team will provide you with detailed and thorough guidelines for your post-surgery care. The typical patient recovery period for eyelid surgery in NYC from Dr. Pearlman is just 1 week.

These are some general guidelines:

  • The first 2 to 3 days are when you’ll experience the most discomfort
  • Using ice packs several times throughout the day will help reduce swelling
  • Get plenty of rest and keep your head elevated
  • Wearing sunglasses can help reduce your healing time
  • Avoid activities that raise your blood pressure

Most patients feel like returning to normal activities within about a week to a week and a half, when most of the bruising and swelling has gone down. If you wear contacts, most patients will  be able to resume wearing them about 2 weeks after your procedure.

Eyelid Surgery in NYC

We often perform a more natural conservation lower eyelid blepharoplasty by preserving the fat that was bulging and creating the bags, then re-positioning that fat pad into the hollow area beneath called the tear trough.
We use your own fat to create a smooth contour. The “hill above” is lowered and the excess placed in the valley that was beneath.

What To Expect

Upper Eyelids

Dr. Pearlman makes incisions in the natural creases of the eyelids to remove excess skin and fat while adjusting muscle tissue. Upper eyelid surgery through blepharoplasty also reduces sagging skin or hooding that may impair vision, fine lines, and puffiness of the upper eyelids

Lower Eyelids

Dr. Pearlman often is able to hide incisions inside the eyelid so no one can ever tell you had surgery. This is called transconjunctival blepharoplasty. We sometimes combine this with laser therapy or an incision just below the lash line of the lower eyelids to remove excess skin. Lower eyelid surgery through blepharoplasty reduces loose skin and fine wrinkles in the lower eyelids as well as puffiness and bags under the eyes.

Why Your Eyes Reveal Signs of Aging Early

They say the eyes are the windows to the soul, but what if you have premature aging around your eyes and it sends the wrong message? The thin skin surrounding your eyes relies on a more delicate internal structure than the rest of your features. Every blink, eye movement, and facial expression requires a range of muscle movements, gradually weakening that already fragile underlying framework.

As we age, our bodies produce less collagen and elastin—natural proteins that play essential roles in maintaining your tissues’ strength and flexibility. Your skin and muscles weaken over time, and the fat pads surrounding your eyes diminish, causing sagging eyelids, under-eye bags, hollow areas under those bags called tear troughs, and other changes around your eyes that can make you appear older, tired, or upset. Repositioning those tissues beneath your skin’s surface with an advanced eyelid surgery in NYC is the most effective long-term solution, as it addresses those concerns at the root.

The Anatomy of the Eyelids

Dr. Steven Pearlman has spent decades studying facial anatomy, and while his expertise remains unparalleled, he understands that everyone’s facial anatomy is unique.

Rather than following a standardized, textbook technique for every eyelid surgery, he applies this vast knowledge to your specific situation, tailoring his approach accordingly.

While every patient has a different eyelid structure and aging process, there are similarities in their anatomy. A blepharoplasty doesn’t just focus on repositioning your eyelid skin, but also the orbicularis oculi muscle responsible for holding your eyes open and the orbital fat pads that can bulge or diminish beneath them. This multi-layered approach is key to delivering the most natural-looking results without appearing overly tight.

A Comprehensive Solution

Prioritizing Preservation Over Alteration

As one of the most delicate plastic surgeries, eyelid surgery in NYC requires unmatched precision, anatomical expertise, and an eye for facial harmony. Blepharoplasty is just as artistic as it is scientific.

At Pearlman Aesthetic Surgery, our mission is to empower beauty. Rather than altering your eyes’ natural structure altogether, Dr. Pearlman focuses on preserving it by anchoring its internal network of tissues into their original, youthful position. To provide the most seamless results, Dr. Pearlman creates the surgical incisions along the natural creases of your eyes for an upper blepharoplasty, and inside of the lower eyelid in other lower blepharoplasty cases, making your scars as inconspicuous as possible.

Blepharoplasties Beyond Anti-Aging

Eyelid surgery isn’t reserved for patients in a specific age range. At Pearlman Aesthetic Surgery, our blepharoplasty techniques offer benefits beyond combatting laxity and other signs of skin aging around the eyes.

Dr. Pearlman performs lower eyelid lifts for patients of all ages struggling with bags and “hollow” tear troughs—the sunken area beneath the eyes. By meticulously repositioning the bulging fat pads, we seamlessly “fill” this area to create a smoother transition from your eyes to your cheekbones, also minimizing dark circles caused by shadows.

While some people develop “hooded” eyelids, others are born with more tissue in their eyelids. An upper blepharoplasty removes excess tissues for lasting results.

Frequently Asked Questions

Blepharoplasty

Are eyelid lift results permanent?

The results of a blepharoplasty performed by expert surgeon Dr. Steven Pearlman can last for years. While lower eyelid lifts often offer permanent results, it’s important to understand that a blepharoplasty does not prevent future aging. Working with our skin specialists to optimize your skin care routine and lifestyle habits is key to maintaining your results from an upper blepharoplasty.

Can I combine an upper and lower eyelid lift?

Yes, combining an upper and lower blepharoplasty can provide more holistic, seamless results for patients seeking a more comprehensive transformation. However, it’s not necessary in every case. Dr. Pearlman will determine if a combination blepharoplasty is your best solution during your consultation.

Typically, Dr. Pearlman recommends this dualistic approach to an eyelid lift for patients struggling with aging in the upper and lower eye—such as drooping eyelids and sunken under-eyes. However, a combined eyelift may also be suitable for younger patients experiencing congenital “bags” and hooded eyelids.

If you are struggling with both the upper and lower areas, only correcting one through a blepharoplasty may leave your features imbalanced, as one area may appear tighter than the other. Performing both procedures simultaneously not only offers more harmonious, natural-looking, and long-lasting results throughout the periorbital area, but also reduces the downtime of undergoing two separate surgeries.

Can I combine my blepharoplasty with other plastic surgery?

Yes! Patients will often elect to receive a blepharoplasty alongside a facelift, brow lift,  lip lift and even rhinoplasty. A facelift only lifts the lower 2/3 of the face (addressing the cheeks, jawline and neck), so combining with a blepharoplasty and brow lift can provide you with total facial rejuvenation.

Does blepharoplasty remove dark circles?

Lower eyelid surgery can remove under eye bags and we can reposition fat to fill in a hollow tear trough. Dark circles caused by this hollowing will disappear after surgery, however if your dark circles are caused by pigment issues in the skin, we’ll need to consider other treatment options.

How can an eyelid lift improve my vision?

As your muscles weaken and your skin loses elasticity and begins to sag, your eyelid skin can create a “hood” that folds over your eye. This can obstruct your vision, especially your upward peripheral sight.

If you need to tilt your head back in order to see properly, excess eyelid tissue is likely the culprit. In this case, undergoing an eyelid surgery in NYC is the only viable solution.

Dr. Pearlman can address both skin and muscle deterioration during a blepharoplasty to provide the most effective results—aesthetically and functionally. By restoring those underlying networks of tissue to their original, youthful position, an eyelid lift grants your orbital muscles more control to keep your eyes open, enhancing your vision and reducing eye strain as a result.

How do I decide between an eyelid lift and a brow lift?

While neither an eyelid lift nor a brow lift is universally better than the other, one procedure may be better for your specific situation, and we can determine this during your consultation. Typically, we recommend lower eyelid surgery in NYC for patients experiencing hollowness or puffiness beneath the eyes, as a brow lift does not address that area.

If you’re struggling with your upper eyelids, we’ll determine the root cause. If your brow muscles and underlying tissues are pushing down on your eyelids—causing “hooding”—we may recommend a brow lift. Should your eyelids have excess skin, muscle, or fat, then an upper blepharoplasty might be a more effective solution.

How long do eyelid surgery results last?

The results of eyelid surgery last for many years. The tissues around the eyes will continue to age naturally over time. Lower eyelid surgery results are often permanent and rarely need to be repeated. Upper eyelid results can last for years with proper care and skincare maintenance.

How should I prepare for eyelid surgery?

Before your eyelid surgery in NYC, you’ll meet with Dr. Pearlman to discuss your situation and create your personalized treatment plan. Here, we’ll also take your “before” photos to reference after your procedure.

After scheduling your surgery, we will provide detailed preparation guidelines, which may include temporarily discontinuing the use of certain medications such as blood thinners. It’s also important to arrange for transportation for the day of your surgery.

How soon can I wear contacts after blepharoplasty surgery?

Most individuals can wear contact lenses about 1 to 2 weeks after surgery.

Is there a non-surgical alternative to a blepharoplasty?

While non-surgical treatments such as laser therapies and aesthetic injectables can address signs of aging around the eyes, they have limitations. For example, fillers and PRF treatments can conceal under-eye “bags” by restoring lost volume, while neurotoxins like Botox can provide a subtle lift to the upper eye area.

However, these alternatives do not provide lasting results and require regular maintenance. If you are struggling with hooded eyelids or laxity that extends your upper eyelid beyond your eye, a surgical eyelift is the only effective treatment.

Dr. Pearlman will help you weigh all of your options—surgical and non-surgical—during your consultation. Keep in mind that pairing injectables and laser treatments can enhance your results from a blepharoplasty, addressing related concerns such as sun damage and balancing your features beyond the eye area.

Is there a non-surgical alternative to eyelid surgery?

Laser procedures and peels can be recommended as alternatives or in combination with eyelid surgery to help address pigment,  fine lines and wrinkles. There is no minimally invasive procedure that can replicate the results of a surgical eyelift.

Lower eyelid bags can sometimes be camouflaged with cheek and lower eyelid filler placed in the hollow below the bags called tear troughs. Botox and facial fillers can be used to address lines and wrinkles, but puffy and baggy eyelids, excess sin, and “hooding” cannot be corrected without surgery

What are the benefits of combining a brow lift and an eyelid lift?

A blepharoplasty pairs beautifully with a range of other cosmetic surgeries, including facelifts and brow lifts. If you’re struggling with skin laxity, or sagging, both on and above your eyelids, a dualistic approach that combines brow and eyelid surgery in NYC may provide more balanced, inconspicuous results. Additionally, you can enjoy longer-lasting results, as Dr. Pearlman repositions the underlying tissues in the entire area to grant it more durable support.

What is the best age to undergo a blepharoplasty?

As with any plastic surgery, there is no universal “right” time to undergo eyelid surgery in NYC. Dr. Steven Pearlman does not rule out candidates who are below the age of 40. Many individuals in their late 30s to mid-40s decide to undergo blepharoplasty for anti-aging purposes, as it’s the general period when your skin’s anatomy significantly changes.

However, Dr. Pearlman performs both upper and lower eyelid lifts to address more than skin aging. Younger patients can also benefit from a blepharoplasty.

Some individuals are born with more tissue in or around their eyelids than others, which can cause eyelid “hooding” and congenital under-eye “bags” at an early age. Beyond the skin itself, your genetics play a role in determining the amount and placement of fad pads. While overly large fat deposits can cause bulging, diminished fat deposits can cause “hollows” that appear dark due to shadowing. Hence why Dr. Pearlman performs blepharoplasties for patients of all ages, although the procedure is more widely known for its anti-aging benefits.

What is the best age to undergo an eyelid lift?

While there is no “best” age to undergo an eyelid lift, most of our eyelid surgery patients are in their 40s or beyond. Some people may decide to undergo the procedure in their 30s and occasionally even in their 20s if they have naturally hooded eyes or premature lower eyelid bags.

What is the cost of eyelid surgery in NYC?

The cost of eyelid surgery in NYC will vary from patient to patient because eyelid surgery can be performed on the upper lids, the lower lids, or both at the same time. If you receive additional treatments such as dermal fillers or laser therapy, then the costs will also naturally be affected. Dr. Pearlman will provide you with a full, detailed quote during your private NYC consultation for blepharoplasty. We will also assist you with securing financing from a variety of reputable healthcare lenders.

What makes a blepharoplasty a complex procedure?

The anatomy of the eye is intricate, and its underlying structures are extremely delicate. A blepharoplasty can improve your appearance and your vision, but it requires a highly skilled hand. Preserving the integrity of these structures is a challenge in itself, and improving them requires even further precision.

A blepharoplasty involves the skin, muscle, and fat—as well as an in-depth understanding of the anatomy of the eye itself. Understanding how these tissues create a network beneath the surface is only the first step to a successful blepharoplasty, and every patient’s anatomy varies.

Beyond anatomical expertise and personalized techniques, your eyelid surgeon must also account for scarring. Your eyes are one of the first things people notice when they look at you, so minimizing scars on and around your eyelids is crucial. Furthermore, your surgeon must also understand the natural healing process and how to expedite it, so you can recover safely and more comfortably.

What should I expect from blepharoplasty recovery?

While recovery timelines vary from person to person—depending on your genetics, lifestyle, and the kind and extent of blepharoplasty performed—most of our patients can return to their routine within 7 to 10 days. Dr. Pearlman will provide detailed recovery instructions and answer any questions you may have, but here is a general guideline for what you can expect.

Before your eyelid surgery in NYC, it’s important to arrange for transportation home from surgery. In the first few days following your procedure, you can expect swelling and bruising to occur around your eyes. Your surgeon will direct you on the kind and amount of pain medications to take during this time to minimize discomfort. It’s important to prioritize rest and to sleep with your head elevated, allowing gravity to play its role in the natural healing process.

Within the next 1 to 2 weeks, the majority of your swelling and bruising will subside. Most patients feel comfortable—physically and emotionally—to return to work and social activities after this period of time, but you may prefer to take additional time to recover. However, if your job requires extensive eye strain, such as using a computer for the majority of the day, your doctor may recommend waiting a bit longer. At this point, you are welcome to use concealer to cover any remaining discoloration, if you please.

After about 4 to 6 weeks from the day of your procedure, you can return to strenuous physical activities. If any swelling remains, it will likely be minimal and resolve itself shortly.
