Nasal hump reduction is the hallmark of rhinoplasty as the most common patient request. Preservation rhinoplasty has recently gained increased popularity as a method for creating a smoother, more harmonious nasal profile while reducing or eliminating the nasal hump.

It accomplishes this by maintaining continuity of the natural in-born bridge or outer portion of the nose, called the dorsum.

Specifically designed to treat and reduce a nasal hump, preservation rhinoplasty in NYC is a new method of achieving a smoother profile with consistent results.
Anatomically, the nasal hump is approximately half bone and half cartilage. Classic rhinoplasty techniques involve a number of methods to reduce the hump by shaving off the bone and cartilage portions of the bridge, or dorsum.

The bone is either rasped down with a bone file, cut off with a chisel, or whittled down with power instruments such as a Piezo Ultrasonic device. The bridge often needs to be reconstructed with grafts. Preservation rhinoplasty eliminates this need.

 

Preservation rhinoplasty techniques work by removing cartilage and bone beneath the outer shell of the nasal bone and cartilage. The bridge is literally then compressed downward, flattening out the hump, creating a smooth, natural, and straight dorsum. The ultimate goal of a preservation rhinoplasty is to leave an undisturbed, smooth nasal dorsum that is now straight and more natural.

Less Invasive

With the technique used for a preservation rhinoplasty, the nasal hump is reduced by removing cartilage and bone under the outer shell of the nasal bone and cartilage. This means that there is often no bone filed, chiseled, or whittled down with instruments from the bridge, creating a less invasive rhinoplasty procedure.

Based upon your unique nasal anatomy, intricacies of the nose, and your own goals, he will then perform a full examination of your nose inside and out. Preservation Rhinoplasty Recovery

Dr. Pearlman and his staff will closely guide you through your post operative care. He will provide detailed instructions on all aspects of the process to help guide your recovery.
The incision heals so that it is barely visible, if at all and only when viewed up close and from below the nose. Dr. Pearlman often says “someone will have to be close enough to be able to count your nasal hairs to see this incision.”

What To Expect

Your first visit to our office will be scheduled for one week post-op, when we remove the cast and any external sutures used (in open rhinoplasty and alarplasty).

In addition, the nose is cleaned and carefully suctioned internally.

Subsequent visits are at 2 and 6 months, and one year post-op for photographs. Most patients will see 85% of their results at 1 month out, 95% of results at 6 months out and 99% at 1 year.

For the first few days, we recommend relaxing as much as possible. Take this time to apply ice to your eyes. After 48 hours, you can catch up on a new show or some reading and just relax.

You are able to fill the post-operative prescription for pain medication before surgery so you are prepared, but a majority of our patients tell us that Tylenol plus Ibuprofen (Advil) together works just fine to control the discomfort. 

If working from home is a possibility, patients usually feel well enough to do so after day 3. You just may want to keep the Zoom camera off, as the cast will still be on and swelling may persist.

Your first visit to our office will be scheduled for 1 week post-op, when we remove the cast and any external sutures used (in open rhinoplasty).

In addition, the nose is cleaned and carefully suctioned internally. 

By this time, day 8, nearly all of any swelling or bruising around your eyes will be gone. There may be a small amount of yellowing underneath the eyes, but this is easily covered with cosmetics. Many patients come to the day 8 appointment and go straight to work at the office after having the cast removed.  

After about 4 weeks, you’ll be able to see 70–85% of your results. Your appearance will continue to improve for up to a year after your procedure.

Follow-up visits are at 2 and 6 months, and 1 year post-op for photographs.

Is a rhinoplasty covered by my insurance?

If the purpose of your rhinoplasty is to correct breathing issues and a deviated nasal symptom, it is possible your insurance may cover the breathing or functional  portion of the procedure! Dr. Pearlman’s dedicated office staff will be more than happy to assist you in discovering whether or not your procedure will be covered. Cosmetic rhinoplasty is not covered by insurance, but we do offer several financing options for you to choose from.

Is preservation rhinoplasty better?

Because of the subjective nature of rhinoplasty based on each patient’s individual anatomy, goals, and needs, we cannot definitely say that preservation rhinoplasty is better. Depending on the patient, preservation rhinoplasty can be a better option if the main goal is to reduce a nasal hump in a less invasive manner and suitable for your specific anatomy.

What does preservation rhinoplasty mean?

Preservation rhinoplasty means that Dr. Pearlman “preserves” the outer nasal bones and reduces the nasal hump by removing cartilage and bone beneath the outer shell of nasal bone and cartilage. Then, the bridge is compressed downward to flatten out the hump for a smooth, straight, and natural-looking dorsum.

What will the first week of recovery entail?

You will have some bruising and swelling around the eyes during the first week of recovery. Some patients need time to adjust to breathing through their mouth; it’s akin to having a mild cold. Fatigue from the anesthesia is common for the first day or two. Patients should rest the first few days in an upright position to minimize swelling for better healing.

When can I return to work?

Most patients can return to work in about 8 days. Bruising may last up to 2 weeks. If you use cover makeup, you may return even sooner. However, patients cannot bend or lift for 3 weeks, so they need to be sedentary at work.
