Teenage rhinoplasty is a suitable option for girls typically starting around age 15 and age 16 for boys. This is the age when their nose and facial features usually stop growing, making them suitable candidates for teenage rhinoplasty. Furthermore, children over the age of 14 are more capable of weighing their options to make a mature decision.

 

Teenagers should also have the mental and emotional maturity and parental support necessary to make such a decision. Dr. Pearlman will begin the consultation listening to your child’s concerns about their nose, specific issues they are looking to resolve, and desired results from teenage rhinoplasty.

Based upon the patient’s unique nasal anatomy, intricacies of the nose, and goals, he will then perform a full examination of the nose inside and out. Using advanced Mirror® 2D and Vectra® 3D imaging, Dr. Pearlman will demonstrate the potential changes to the nose to help envision future results.

Teenagers are very visual and the use of this technology is vital to understanding not only the potential changes and improvements to the nose itself, but also to visualize the new nose and how it blends with the rest of the patient’s facial features.

Lastly, you and Dr. Pearlman will establish mutual goals to enhance the appearance and breathing function of the nose.

Dr. Pearlman makes incisions to provide access to the underlying structure of the nose. A closed or scarless rhinoplasty technique places all the incisions inside the nostrils, so there is no visible scarring. Dr. Pearlman uses this for the majority of his primary rhinoplasties.

An open technique places a tiny incision on the base of the nose between the nostrils. This incision heals so that it is barely visible, if at all and only when viewed up close and from below the nose. Dr. Pearlman often says “someone will have to be close enough to be able to count your nasal hairs to see this incision.”

Dr. Pearlman will discuss the reasons for choosing incisions at the time of your consultation.

Dr. Pearlman removes, reshapes, and adds cartilage where needed to correct the appearance of the nose. He removes wide nasal bony humps with precision micro instruments. Removing cartilage and suture contouring corrects a large nasal tip.

Reshaping cartilage and adding support using the patient’s own cartilage can address a drooping nasal tip. Form and function go together in rhinoplasty. This is what Dr. Pearlman stresses when lecturing around the world. He often adds grafts of the patient’s own cartilage to maintain or enhance nasal support during cartilage shaping.

Once Dr. Pearlman properly shapes the nose, he closes the incisions. Dr. Pearlman places nasal packing and splints to help support the nose while it heals. He no longer uses nasal packing in the vast majority of patients that significantly reduces postoperative discomfort. Finally, he places an external cast splint on the nose to be left on for one week.

Dr. Pearlman and his staff will closely guide you through your post operative care. He will provide detailed instructions on all aspects of the process to help guide your recovery.

What To Expect

Your first visit to our office will be scheduled for one week post-op, when we remove the cast and any external sutures used (in open rhinoplasty and alarplasty).

In addition, the nose is cleaned and carefully suctioned internally.

Subsequent visits are at 2 and 6 months, and one year post-op for photographs. Most patients will see 85% of their results at 1 month out, 95% of results at 6 months out and 99% at 1 year.

What is the advantage of computer imaging?

Dr. Pearlman utilizes Mirror computer imaging and VECTRA 3D imaging technology during his consultations for all primary rhinoplasty patients, including for teenage rhinoplasty consultations. Mirror and VECTRA 3D imaging allow Dr. Pearlman to manipulate a “before” photo of the patient’s face to visually demonstrate the effects of the rhinoplasty procedure. This technology allows you to preview an approximation of what the nose will look like after surgery so you and Dr. Pearlman can decide what techniques will best suit your needs.

What will the first week of recovery entail?

Your child will have some bruising and swelling around the eyes during the first week of recovery. Some patients need time to adjust to breathing through their mouth; it’s akin to having a mild cold. Fatigue from the anesthesia is common for the first day or two. Patients should rest the first few days in an upright position to minimize swelling for better healing.

Will there be any lasting scarring?

If Dr. Pearlman uses a closed, scarless rhinoplasty technique, there won’t be any visible scarring. If he uses an open technique, there will be a tiny incision at the base of the nose between the nostrils, but this incision will likely fade over time and is small to start with. It is often undetectable to the naked eye. During your consultation, Dr. Pearlman will discuss which technique is most appropriate for your unique nose.
