General

Since scars represent significant damage to the skin, you will need new collagen product to reduce or eliminate them. The three most common types of acne scars are ice pick scars, boxcar scars, and rolling scars. Depending on the type of scar you have, we will customize your SkinPen® treatment to meet the individual scar formations on your skin. Your treatment settings are unique and often varying depths are utilized to be more aggressive in some areas, and less aggressive in others. This is truly couture skincare!

At Pearlman Aesthetic Surgery, your microneedling in NYC experience is always rooted in precision, tailored to your needs, and centered on an elevated understanding of your skin’s biology. We understand that every patient arrives with their own unique concerns, including textural irregularities and scar patterns.

Because of this, our treatments comprise a customized approach rather than a one-size-fits-all protocol. Before treatment, our expert clinicians conduct a comprehensive skin analysis. They assess any general concerns you may have about your skin, including scar type, dermal thickness, skin elasticity, and overall skin health. This keeps your microneedling journey evidence-based and optimized for the best possible results.

Patients often notice subtle yet meaningful improvements in their skin’s texture, firmness, and radiance in the weeks following microneedling treatments. Results continue to evolve over the following weeks and months after your procedure.

Alongside the health of your skin, your provider at Pearlman Aesthetic Surgery will also consider your overall aesthetic goals, lifestyle considerations, and any additional therapies that may complement your microneedling in NYC.

Microneedling using the SkinPen® stimulates the body’s natural wound-healing process, promoting fibroblasts within the skin to produce new structural proteins. It also encourages collagen and elastin restructuring within the dermis. This process enhances the skin’s resilience and can improve overall tone and texture.

For some patients, a combination approach promotes rejuvenation and even more skin texture refinement. Your provider will consider integrating targeted topical serums, IPL (Intense Pulsed Light) facials, or injections. This multifaceted strategy customization allows you to receive a care plan that is as individualized as your fingerprint.

Our team utilizes advanced skin-soothing protocols, topical anesthetics, and specialized techniques to ensure your visit is both comfortable and results-driven. Your provider will recommend personalized post-treatment care to support your optimal healing and to prolong the benefits of your SkinPen® treatments.

Microneedling at Pearlman Aesthetic Surgery represents a meeting of clinical insight, expert judgement, and state-of-the-art technology to deliver enduring results tailored uniquely to you.

Ice Pick Scars

Ice Pick scars are the most common type of scarring caused by acne, they are characterized by their hole-like shape left from an ice pick. Ice Pick acne scars are deep and narrow, which makes it difficult for them to be filled up naturally with collagen, and some are too deep to heal entirely. These acne scars are usually less than 2mm across and caused by inflamed comedones (blackheads) or acne vulgaris (whiteheads).

Ice Pick scars present a unique challenge in aesthetic care because of their shape and orientation within the skin. Their narrow profile often extends into the deeper layers, beyond the outer layer of the dermis.

Due to this placement, Ice Pick scars typically require a targeted, precision-driven treatment to stimulate visible remodeling. They do not respond easily to surface-level treatments alone.
Understanding the structural complexity of this type of scarring is essential for tailoring an effective correction strategy. At Pearlman Aesthetic Surgery, we consider the underlying properties of Ice Pick scars, the quality of the skin surrounding them, and each patient’s unique skin profile when developing a treatment plan.

Because the skin’s collagen production diminishes with age and exposure to the elements, microneedling in NYC works to activate fibroblasts and trigger controlled wound healing. This helps microneedling be an especially powerful treatment for addressing this deep type of scarring.

Over time, a tailored microneedling treatment plan can progressively soften the sharp edges of Ice Pick scars and improve surface regularity. Customized adjustments in needle depth allow your provider to target the deeper elements of these scars while preserving surrounding tissue integrity.

For patients with particularly deep Ice Pick scars, especially scars that have resisted past treatments, our team may recommend ‌combination therapy. Your provider can integrate other treatments available at Pearlman Aesthetic Surgery into a comprehensive microneedling plan to optimize results. Some key treatments work together to enhance dermal thickening, improve skin strength, and promote a more uniform textural appearance.

Microneedling, when done using advanced devices like the SkinPen®, creates uniform microchannels in the skin that extend through the epidermis into the upper dermis. Each controlled micro-injury promotes a strong repair response within the skin while encouraging the body to produce additional collagen and elastic fibres.

Ultimately, treatment for Ice Pick scars aims to restore the physical architecture of the skin while also improving a sense of confidence in your complexion. Through individual care plans and our state-of-the-art approach, individuals can see meaningful and lasting improvements with microneedling.

Boxcar Scars

Boxcar acne scars are like Ice Pick scars, however the Boxcar scars are wide and more rectangular with vertical edges. Boxcar scars are usually found on the temples and cheeks, and they can be deep or shallow. The majority of Boxcar scars are not as deep as Ice Pick scars.

The broader, larger shape of Boxcar scars creates a different textural challenge than other, more narrow scar types. Because of their defined edges and wider surface area, these scars can create noticeable dips in the skin that cast shadows and interrupt the smooth contour of the face.

While shallower Boxcar scars may sometimes respond to mild exfoliation or light resurfacing, deeper scars often require more robust regenerative strategies to achieve meaningful and long-lasting improvements.

At Pearlman Aesthetic Surgery, our treatment for Boxcar scars begins with a comprehensive assessment of scar depth, distribution, and the overall condition of the surrounding skin.

Every patient’s skin is unique, and by understanding both the visible scar and the health of the underlying dermis, our team can tailor a treatment plan for safe and effective remodeling. Microneedling in NYC plays a central role in this strategy by triggering your body’s repair mechanisms in a controlled and precise manner.

Microneedling application creates tiny, uniform channels in the skin. These micro-injuries trigger the body’s healing response, stimulating the production of new collagen and elastin to rebuild skin.

These key proteins and structures support the skin’s health and smoothness while improving its strength. Over a series of treatments, this process can soften the appearance of Boxcar scars, improve contour irregularities, and enhance the skin’s overall texture.

Because Boxcar scars vary in depth across the face, your provider will adjust the needle depth during the session to treat each area evenly. Due to the varying depth and the potential to be fairly deep, combining microneedling with treatments like light chemical peels, tailored skincare products, or laser rejuvenation can further improve the appearance of these scars.

Your provider will recommend additional treatments based on your skin type, sensitivity, and aesthetic goals, keeping your personalized treatment protocol effective, comfortable, and safe.

The goal of treating Boxcar scars is to restore a more even surface while promoting long-term skin rejuvenation. With personalized care and advanced regenerative techniques, many of our patients experience visible improvements that enhance not only their skin’s appearance but also their confidence.

Rolling Scars

Rolling scars are the least common of the three, typically they are shallow and wide. Damage causes rolling scars under the uppermost surface of the skin, they are round and give the skin a rolling, wavelike appearance.

Because Rolling Scars sit partially beneath the dermis of the skin, effective treatments must address both the visible texture and the underlying skin structure. Superficial treatments alone rarely create lasting, visible improvements in this scar type when used on their own.

Instead, therapies that stimulate deeper collagen production and encourage skin remodeling deliver the most meaningful results. This is why our team often integrates microneedling in NYC into Rolling Scar treatment protocols—because it helps restore smoother, more even-looking skin.

Microneedling’s effectiveness lies in its ability to deliver controlled micro-injuries to the skin. These micro-injuries activate the skin’s natural regeneration and healing response, encouraging the production of new collagen and elastin. This works to strengthen the dermal layer of the skin, improving its overall health and firmness.

Over time, as collagen rebuilds, the depth of Rolling Scars become less pronounced, allowing the dermis of the skin to appear smoother and more refined. Rolling scars often vary in depth across different areas of the face, so your provider will carefully adjust the treatment setting of your microneedling protocol to target each specific region effectively.

At Pearlman Aesthetic Surgery, our team tailors treatment for rolling scars to each patient’s unique skin type, health, and concerns. Our team will carefully evaluate factors such as skin thickness, scar distribution, age, and overall skin texture before proceeding with treatment.

In some cases, our team may recommend combining microneedling with other supportive treatments or professional-grade skincare to enhance results and promote long-lasting skin healing and health.

With consistent treatment and proper aftercare, many patients experience gradual yet noticeable improvement in the appearance of their rolling scars. Beyond improving texture, microneedling in NYC can also enhance the skin’s overall quality, leading to brighter, healthier-looking complexions and improved confidence.

Microneedling

How it Works

Consultation

Procedure

Recovery & Downtime

We typically recommend a series of 3–5 treatments for optimal results.

Each treatment will induce some collagen remodeling, and we can use microneedling for ongoing skin maintenance.

In your initial consultation, your provider takes the time to thoroughly assess your skin and customize a treatment plan tailored to your individual concerns and goals. Because every patient has unique skin, our protocols are personalized rather than one-size-fits-all.

During this first visit, we discuss your medical history, review any previous aesthetic or skin care treatments, and work to set realistic expectations for outcomes based on your specific skin profile.

Microneedling in NYC stimulates the body’s natural healing response, promoting collagen and elastin production over time. These critical structural proteins in the skin support healthy, young-looking skin over time.

During your consultation, your provider will evaluate lifestyle factors, such as sun exposure, skincare routine, and any habits that may affect skin quality. This enables us to recommend and tailor supportive care between sessions.

Maintenance appointments help preserve the results achieved by the initial microneedling treatment. While many patients notice subtle improvements after even a single session, the cumulative effects of a microneedling treatment series provide the most significant and long-lasting enhancements.

Depending on your skin’s individual response, maintenance sessions are spaced several months apart. Throughout your treatment journey, the Pearlman Aesthetic Surgery team prioritizes your comfort, safety, and results. We are here to guide you at every step toward healthier, more radiant skin.

Treatment time will vary depending on the surface area treated but typically will range from 15 to 30 minutes.

Because we apply topical anesthetic ahead of time; plan to spend between 30 and 60 minutes in the office.

During your microneedling in NYC, you receive care in one of our luxurious treatment rooms. Before starting any protocols, your provider will apply a topical anesthetic to your targeted treatment area(s).

This helps to minimize potential discomfort throughout the treatment process. We allow the numbing agent to sit on the skin for several minutes to take full effect, so the total in-office time for this treatment is longer than the actual microneedling treatment itself.

After adequate preparation, your provider will carefully pass the microneedling SkinPen® device over the dermis of the skin. Your provider uses precise, controlled movements to ensure consistent coverage and depth based on the specific needs and goals outlined during your one-on-one consultation. The treatment pacing is deliberate, as your provider stimulates collagen and elastin production while maintaining the highest standards for safety and comfort.

Immediately after a micro-needling treatment the skin can appear flushed like a light sunburn to deeper pink with pinpoint bleeding for deeper treatments.

Typically, any redness will resolve within 1 or 2 days. You can apply make-up immediately if desired. You can use ice packs or cool compresses to soothe the skin. Practice good sun protection and avoid abrasive topicals or treatments for 2 to 3 days.

Your skin will need multiple treatments to get the best results from SkinPen®. Through microneedling, you will notice smoother overall skin texture and a reduction in the appearance of scars.

In the hours following your initial microneedling in NYC treatment, it is also common for the skin to feel tight, warm, or slightly sensitive. This natural response indicates the active healing process that is triggered by microneedling, and it signals that the body has enhanced collagen production.

Most of our patients find these sensations mild and tolerable, and these sensations improve on their own in the first 24 hours after treatment.

Post-microneedling skin hydration plays a critical role in recovery. We recommend using gentle, fragrance-free moisturizers to support the skin’s barrier as it repairs and restores itself. Avoid active ingredients such as retinoids and exfoliating acids until your skin has fully recovered, usually about a week after your treatment. Please check with your provider when you can resume your skin routine.

Additionally, patients should prioritize sun protection after microneedling. Freshly treated skin is more sensitive to UV exposure and damage. Apply a broad-spectrum sunscreen throughout the day, and limit direct sun exposure to protect your investment and maintain an even skin tone.

Over the several weeks following microneedling in NYC, your body will continue to produce more collagen and elastin to add support to the skin’s layers. Because this process takes time, improvements will develop gradually and become more notable with each supporting treatment session.

With a consistent treatment plan and intentional aftercare, microneedling offers refined, natural-looking enhancement to the skin that can reduce the appearance of texture while rejuvenating from within.

Frequently Asked Questions

Microneedling

Are there any contraindications for a SkinPen® treatment?

You should not get micro-needling if you have vitiligo, an active skin infection (such as flat warts or a cold sore), or if you’ve taken isotretinoin within the past 6 months. Please notify your provider if you are prone to excessive/keloid scarring or have a collagen vascular disease such as lupus.

Does the SkinPen® treatment hurt?

Deeper micro-needling treatments can be uncomfortable, so we typically apply a topical anesthetic ahead of time to reduce any discomfort. More superficial treatments are well-tolerated with no anesthetic.

How is microneedling with SkinPen® different from a laser resurfacing treatment?

Unlike lasers, SkinPen® breaks up pigment mechanically and remodels the collagen without heat. This can make the treatment safer and more comfortable for some patients.
