This procedure utilizes “short flap” incisions that begin just above the ears, go down and around the inside of the small rectangular “tragus” at the opening of the ears, and end just behind the earlobes.

SMAS Facelift (Rhytidectomy)

A SMAS facelift gets its name from a layer of tissue beneath the skin–the superficial muscular aponeurotic system. Through the aging process, this layer gradually loses support and elasticity, leading to drooping cheeks, pronounced jowls, deeper nasolabial folds, and sagging tissues in general.

A SMAS facelift addresses these issues at the source, lifting and tightening the underlying musculature before trimming excess fat and skin. During your consultation for facelift surgery in our New York office, the goal is absolute understanding between surgeon and patient. Dr. Pearlman will thoroughly assess your specific aesthetic needs.

How much lifting do you need? Do you have banding of the platysma muscle under the neck? Will a brow lift or eyelid procedure help rejuvenate the upper face? Which method will give you a refreshed and younger appearance? He analyzes your entire face with rejuvenation as the goal, and natural-looking results as the promise.

Mini Lift

We conduct the mini-facelift surgery under general or local anesthesia. Dr. Pearlman will make small hairline incisions around the ears to “elevate ” your skin upwards, redistribute fat and muscle, and trim excess skin tissues.

Once the procedure is done, your incisions will be sutured closed, and you’ll be released after a brief evaluation period.

Male Facelift

A difference of male facelifts that may occur is incision placement. Women generally have longer hair capable of covering the light incision scars. The same can’t be said for men. Placing incisions in the same style and location would lead to greater visibility after the procedure. Dr. Pearlman works with your unique appearance to fashion a tailored face lift.

During the procedure, Dr. Pearlman will place an incision that begins in the hairline, extends behind the ear, and into the hair. Your skin is lifted to isolate it from the underlying SMAS layer, and the underlying tissues are trimmed and manipulated to produce the desired results, and the facial skin is then lifted upwards.

Fat deposits are removed, and the skin is trimmed to produce a taut and smooth appearance. Finally, the incisions are sutured closed, and you can begin the recovery process.

A deep plane facelift uses the same incisions as a “short-flap facelift,” with another extension extending to the hairline behind the ears and into the temple. The technique involves lifting the underlying supporting tissues along with lax, drooping, or sagging skin.

The SMAS muscle is released from connections to the facial skeleton beneath to get maximal mobility and eliminate tension. Dr. Pearlman repositions any laxity at the cheeks and upper neck is improved, as the skin, SMAS and supporting tissue upward and back toward the ears. This unique facelift technique produces remarkably natural-looking results.

Considerations

Many patients have questions or concerns about anesthesia, and this is understandable. Dr. Pearlman typically performs a facelift with general anesthesia, meaning the patient is fully sedated during the procedure.
Dr. Pearlman employs only the finest surgical staff to support him during surgery, including a medical doctor as his anesthesiologist. At Pearlman Plastic Surgery, we have a pristine safety record for our patients, and that includes our application of anesthesia.

The Facelift Recovery Process

Unless you receive a mini-facelift, facelift patients must stay overnight in our facility with our nurse. Dr. Pearlman will advise you to stay indoors and avoid activities for a few days, and he’ll provide a set of detailed and thorough recovery guidelines.

You will need to plan for time to recover, which varies by individual and the type of facelift. You will need to plan to rest for a week or two before resuming normal activities.Gradually, the redness, bruising, and swelling will fade. The most challenging part of recovery is during the first week, due to the swelling and bruising. During this phase, you will have medication to help keep you comfortable.

Step 1.

After surgery, we often use drains in the neck to remove any blood that may collect and prevent it from accumulating under the skin. These drains are removed the morning after surgery before the patient returns home.

Step 2.

Six days post-op, we remove sutures in front of the ear where the skin is thinner.

Step 3.

Eight days post-op, we remove the sutures along the scalp where the skin is thicker.

Step 4.

Approximately a week and a half post-op, most bruising has gone away. Until this point, such bruising can typically be covered with makeup.

Step 5.

One and a half to two weeks post-op, most patients are able to return to work if they don’t mind that some swelling is still present.

Step 6.

At two weeks post-op, most patients are presentable for social situations.

Step 7.

Between three weeks to a month, patients can resume almost all functions. For very important functions, such as a wedding of a close family member, Dr. Pearlman recommends at least two months of healing.

What’s the difference between a deep plane facelift and a traditional facelift?

Will there be visible scars?
